About this Talk
The REACH team based in North Yorkshire is a multidisciplinary, multi-agency partnership team consisting of Housing, Mental Health, holistic support and domestic violence colleagues and works in connection with key partners offering a housing first approach to those who are homeless with complex needs.
This session will provide an overview of this multi-agency Housing First model including an explanation of funding channels for the team and future plans.
They will then demonstrate a multi-agency group supervision involving the whole REACH team and complete a meta-reflection of themes discussed inviting views and reflections from those in the room.
